You will need the following information to complete this nomination form:

1. Student(s) name
2. Student(s) email address
3. Student(s) phone number
4. Student(s) guardian name, email address, and phone number
5. At least one alternate delegate with the information above

To ensure successful registration, please follow these two registration tips:

• Please use the student's personal email address, rather than a school-issued email address, if possible.
• Please also enter a unique email address for each student nominated to ensure each student receives a registration email and unique Delegate ID Number.
